Position: Community Support Specialist

Key Responsibilities:

Provide personalized support and resources for individuals with aging-related challenges and disabilities in the community.
Develop and implement tailored care plans to enhance quality of life for our clients.
Collaborate with interdisciplinary teams to ensure comprehensive and holistic support for individuals in need.
Facilitate educational workshops and training sessions to empower clients and caregivers.

Qualifications:

Bachelor's degree in a related field (social work, psychology, human services, etc.)
Strong communication and interpersonal skills
Empathy, compassion, and a commitment to advocacy
Experience in community outreach and service delivery preferred
