Responsibilities:
- Develop and maintain relationships with community partners and stakeholders
- Coordinate support services for individuals and families in need
- Assess client needs and develop personalized care plans
- Advocate for clients to ensure they receive appropriate services and resources
- Provide guidance and support to a team of care professionals
Requirements:
- Bachelor's degree in social work, psychology, or related field
- Proven experience in community outreach and support services
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
