As a Community Care Specialist, you will play a crucial role in delivering personalized care and assistance to those in need while being an integral part of our dynamic team.

Responsibilities:

Conduct home visits and assessments for patients in the Zululand area
Develop personalized care plans and provide ongoing support to patients
Collaborate with healthcare professionals and community resources
Participate in educational programs and outreach initiatives

Requirements:

Relevant experience in patient care and support
Strong interpersonal and communication skills
Ability to work independently and as part of a multidisciplinary team
