As a Community Care Specialist, you will play a crucial role in delivering personalized care and assistance to those in need while being an integral part of our dynamic team.
Responsibilities:
- Conduct home visits and assessments for patients in the Zululand area
- Develop personalized care plans and provide ongoing support to patients
- Collaborate with healthcare professionals and community resources
- Participate in educational programs and outreach initiatives
Requirements:
- Relevant experience in patient care and support
- Strong interpersonal and communication skills
