Position: Veteran Care Coordinator
Job Description:
- Assess and coordinate healthcare services for veterans
- Develop individualized care plans based on veterans' needs
- Coordinate with medical professionals to ensure appropriate care and treatment
- Advocate for veterans' benefits and entitlements
- Provide emotional support and guidance to veterans and their families
Requirements:
- Bachelor's degree in a healthcare-related field
- Experience working with veterans or a population in need
- Strong communication and organizational skills
- Ability to work independently and as part of a team
- Understanding of veterans' healthcare services and benefits
