Position: Veteran Care Coordinator

Job Description:

Assess and coordinate healthcare services for veterans
Develop individualized care plans based on veterans' needs
Coordinate with medical professionals to ensure appropriate care and treatment
Advocate for veterans' benefits and entitlements
Provide emotional support and guidance to veterans and their families

Requirements:

Bachelor's degree in a healthcare-related field
Experience working with veterans or a population in need
Strong communication and organizational skills
Ability to work independently and as part of a team
Understanding of veterans' healthcare services and benefits
