We Are Seeking a Community Wellness Navigator

About the Role

As a Community Wellness Navigator, you will serve as a crucial link between our community members and healthcare resources. Your primary responsibility will be to guide individuals towards resources that promote holistic well-being, including physical, mental, and emotional health support. This role requires a compassionate and empathetic approach, as you'll be working closely with individuals from various cultural backgrounds and walks of life.

Key Responsibilities

Build strong, trusting relationships with community members
Facilitate access to healthcare services and information
Advocate for culturally sensitive care and support programs
Create and implement wellness initiatives tailored to community needs
Collaborate with local healthcare providers and organizations to bridge gaps in care

Qualifications

Strong understanding of community wellness and healthcare resources
Excellent communication and interpersonal skills
